Short Term Rehabilitation

Short term rehabilitation often refers to the post-acute stay at a skilled nursing center following a surgery, illness or injury. It is common for people to leave the hospital but not be ready to return home.

Common reasons why seniors may need short term rehabilitation include:

  • Orthopedic injuries such as fractures or joint replacements
  • Wound care
  • Acute medical conditions
  • Interim medical care following a hospital stay
  • Respiratory care
  • Neurological conditions such as Parkinson's disease
  • Oncology recovery
  • Stroke recovery
  • General weakness or physical decline
  • Diabetic rehabilitation

Our team of professionals help people transition back to their home safely and successfully. We complete in-home assessments prior to leaving the short term care center to evaluate areas that need greater attention or to facilitate the process for getting adaptive equipment in the home. Most patients graduate to their home environment within 4-6 weeks.

Our short term rehabilitation centers are Medicare-certified. In addition, we accept most managed care and commercial insurance, as well as private payment.
